Yep, those two can be a real doozy, but I forget what other alternatives there are (other than 'Off belay', of course). I am sure it has happened... when someone screams out 'Take' about to peel off (fall), only to hear your belayer say 'Off belay' who thinks you have topped out and are 'safe'...
